1. Physician time is $210 per Hour, and your Initial Evaluation could take an hour or more.

2. A deposit of $210, a week before your appointment, is required to reserve an Initial Evaluation.

3. Follow up visits are billed in 15 minute increments

4. There is a $25 dollar charge for returned checks, and there is $50 charge for missed appointments without a 24-hour cancellation.

Our financial policies have been established to assure you and your family the best possible service and to avoid misunderstandings which might interfere with treatment. Our professional services are rendered to the patient and not the insurance company. We are unable to bill insurance companies; however we will provide you with a itemized bill that you may submit to your insurance company. You simply need to send in the forms to your own company.

Telephone appointments are sometimes given for your convenience if you have difficulty getting here as frequently as we would like, and if you are progressing well. A Credit card # or a payment in advances due at time of Phone appointments.
